Key Responsibilities:
• Perform assessments and evaluate care requirements
• Assign clients to appropriate staff for care navigation

• Support Aging in Place and Home First philosophy
• Ensure culturally secure and inclusive settings
Requirements:
• Registered Nurse licensed with YRNA or equivalent
• Recent experience in coordinating complex client services
