This Full time role is critical in the Post Stroke Transitional Care Program, where you will help clients regain independence after a stroke. You're tasked with developing and implementing personalized care plans and ensuring ongoing client support. With a breadth of responsibilities, your organizational and interpersonal skills will drive positive client outcomes.
Key Responsibilities:
• Implement individualized support services plans for stroke recovery
• Deliver services with flexibility during business hours




• Maintain accurate daily logs and create service development reports
• Participate in ongoing training as necessary
• Follow established policies and procedures rigorously
Requirements:
• Post-Secondary diploma in relevant fields preferred
• Minimum three years’ experience in Human Services
• CPR and First Aid certification is a plus
• Valid driver’s license and vehicle for business use
• Ability to work independently and problem-solve effectively
