Key Responsibilities:
• Engage with unattached patients proactively
• Complete patient onboarding and chart preparations
• Facilitate warm exchanges between patients and care teams

Requirements:
• Registered Practical Nurse in positive standing with the CNO
• Two years of experience in clinical or community nursing
• Experience in electronic medical records beneficial
• Solid clinical independence and assessment abilities
