This role requires a Registered Nurse dedicated to supporting patients and families in need of palliative care. Working within the Northern Health Palliative Care Consultation Team, you will engage with long-term care, acute, and community teams to provide essential guidance. Your expertise in education, project management, and palliative care will be key in coordinating and evaluating care services effectively.
Key Responsibilities:
• Plan and organize palliative care services in various settings
• Teach and mentor primary caregivers in effective practices
• Collaborate with interdisciplinary teams to enhance patient care
• Evaluate pain and symptom management strategies
• Conduct quality improvement initiatives for service enhancement
Requirements:
• Registered Nurse with BCCNM certification
• Experience in palliative care settings preferred
• Solid organizational and communication skills
