As a full time Youth Prevention Outreach Worker, you will empower youth aged 12–24 who are at risk of gang involvement or criminal activity. Utilize your skills in trauma-informed care, community navigation, and relationship-building to create a positive trajectory for underserved individuals. Collaborate with a supportive wraparound team to ensure no young person falls through the cracks.
Key Responsibilities:
• Provide relationship-based support in schools and communities
• Conduct strengths-based assessments for individual success
• Connect youth with counseling and education resources
• Support youth transitioning from detention to community
• Deliver gang prevention education and healthy decision support
Requirements:
• Degree in Social Work or related field
• Experience in case management with at-risk youth
• Competency in Arabic, Somali an asset
• Legal right to work in Canada
• Adaptable to work evenings/weekends
