Support adults with disabilities as a Community Support Worker at The Salvation Army. Focus on life skills, education, and promoting independence through personalized community-based support.
In this role, you will provide tailored assistance to individuals, facilitating their journey towards achieving personal goals. You will collaborate with team members while ensuring the highest quality of care related to physical, emotional, and medical needs. This position requires flexibility in scheduling and will involve working closely with Service Users in various community settings.
Key Responsibilities:
• Design and implement individual support plans for Service Users
• Provide personal care, including lifting and transferring
• Assist with goal-setting and achieving desired outcomes
• Coordinate secure transportation for Service Users
• Maintain accurate records of daily activities using software
Requirements:
• Two years’ experience with developmental disabilities
• Completion of a relevant diploma in Human Services
• Valid Driver's License and vehicle insurance
• Current First Aid and CPI certification
• Satisfactory Criminal Records Check
