In this role, you will work under the supervision of the Patient Care Manager, focusing on the rehabilitation of older adults after significant health challenges. Your responsibilities will range from delivering patient care to assisting with admissions and transfers. You'll also engage with the interdisciplinary team in care planning and evaluations, advocating for your patients throughout the process.
Key Responsibilities:
• Provide nursing care that meets established care plans
• Administer medication and monitor patient responses
• Conduct assessments and participate in care conferences
• Support patient education and discharge planning
• Perform clerical duties and assist with staff training
Requirements:
• Graduation from a recognized Practical Nursing program
• Valid registration as an LPN in British Columbia
• Current basic cardiac life support certification
• Experience in CPR and nursing care procedures
• Physical capacity to fulfill nursing responsibilities
