In this role, you will use your personal experience to offer case management to a caseload of 15-20 clients. Your responsibilities include conducting needs assessments, community connections, and supporting clients in accessing essential resources. You'll also collaborate closely with community partners to ensure holistic client support.
Key Responsibilities:
• Manage peer support for a caseload of clients
• Maintain accurate records and logs
• Facilitate connections with clients and their loved ones
• Engage in community meetings for information gathering
• Accompany clients to appointments as needed
Requirements:
• Degree or diploma in Social Services/Human Services
• 2+ years of experience in social services
• Knowledge of local community resources
• Solid problem-solving and de-escalation skills
• Valid driver’s license and vehicle necessary
