In this permanent role, your focus will be on performing comprehensive assessments and creating tailored care plans within a supportive interdisciplinary team. You'll play a crucial role in educating clients and mentoring fellow caregivers, ensuring the highest standards of care are provided. Key Responsibilities:
• Perform comprehensive clinical assessments
• Implement individualized care plans collaboratively
• Mentor and educate clients, families, and staff
• Participate in virtual interdisciplinary consultations
• Lead care plan discussions with clients
Requirements:
• Active Registered Nurse license in Ontario
• NSWOCC completion or equivalent education
• Preferred experience in community and acute care
• Robust assessment and teaching skills
• Excellent written and verbal communication
