Reporting to the Patient Care Manager for Emergency Mental Health, the Addiction Counsellor II will collaborate within the Mental Health and Addictions Program. Key responsibilities include offering support and navigation to adult and youth patients with substance use. You will work closely with ED teams, community service providers, and other healthcare specialists to ensure evidence-based, patient-centered care.
Key Responsibilities:
• Maintain relationships with ED and addiction service programs




• Identify resources to support patient navigation through complexities
• Provide community education on available services
• Prepare individualized treatment plans based on assessments
Requirements:
• Undergraduate degree in Social Sciences
• Experience with substance use care and case management
• Knowledge of harm reduction and concurrent disorders
• Excellent interpersonal and organizational skills
