Mosaic Primary Care Network is looking for a Community Intake Coordinator for its Mental Health Program in Calgary. This casual role is vital for supporting patients through the intake process for mental health services.
As the first point of contact for patients, the Intake Coordinator gathers essential information and addresses patient queries to enhance the referral experience. This position works closely with a multidisciplinary team to streamline the intake and coordinate timely mental health support offered by Mosaic PCN.
Key Responsibilities:
• Guide patients through clinical intake assessments
• Perform routine psychosocial and suicide risk screenings
• Schedule appointments with mental health practitioners




• Maintain accurate patient records in EMR systems
• Engage in community activities to promote services
Requirements:
• Minimum of 2 years clinical experience in mental health
• Relevant Social Work Diploma or bachelor’s degree
• Active registration with a qualified body
• Certificates in crisis intervention or mental health
• Effective communication and teamwork skills
