As a Registered Nurse, your primary focus will be to support individuals with reliable mental illness through comprehensive nursing care and community-based rehabilitation. Collaborating with an interprofessional team, you will assess client needs, conduct health monitoring, and educate clients on medications and symptoms. Your experience in case management and ability to work independently will be invaluable in providing high-quality mental health support.
Key Responsibilities:
• Provide personalized nursing care and health education
• Monitor client mental status and physical health
• Foster relationships through therapeutic interactions
• Connect clients to appropriate services and resources
• Contribute to health care and chronic disease initiatives
Requirements:
• Valid Ontario nursing registration
• At least two years in mental health care
• Experience with community health preferred
• Knowledge of ACT Model and recovery practices
• Must possess a valid driver's license
