As a Community Health Worker at UHN, you will collaborate with peer workers and community organizations to address the needs of clients. Key Responsibilities:
• Complete social issue assessments for patient support
• Educate clients and families about resources
• Facilitate engagement in community health services

Requirements:
• Bachelor’s degree in health or social work
• Minimum 2 years of relevant experience
• Solid knowledge of community resources
• Experience in supportive housing environments
• Excellent communication and interpersonal skills
