Flemingdon Health Centre seeks a full time Manager, Primary Care Transformation until March 31, 2027, with potential for extension. This role involves leading key projects in the Primary Care Network, focusing on interprofessional team strategies and enhancing healthcare accessibility. You will excel in project management while coordinating with partners to ensure clarity and progress in your initiatives.
Key Responsibilities:
• Lead PCN/IPCT projects from start to finish
• Prepare project materials including summaries and updates
• Maintain project documentation and accountability logs




• Coordinate meetings and support working group activities
• Engage primary care providers for membership and collaboration
Requirements:
• Undergraduate degree in Health Administration or related field
• Minimum four years’ experience in project management
• Solid interpersonal and communication skills
• Experience in healthcare settings preferred
• Ability to manage multiple priorities effectively
