This Clinical Social Worker role is integral to either the Urgent and Primary Care Centres or Primary Care Clinics. You will assess client needs, collaborate with healthcare specialists, and ensure quality care for vulnerable populations while facilitating community resource access.
Key Responsibilities:
Evaluate psychosocial needs through interviews and assessments
Deliver therapeutic interventions for mental health challenges
Work with teams to design personalized care plans
Support seamless transitions from acute to community care
Organize and document case management activities
Requirements:
Active registration with BC College of Social Workers
At least two years of relevant health care experience
Solid understanding of counseling frameworks and regulations
Valid driver's license and vehicle access
Ability to thrive in a diverse team workplace
