Enhance senior care as a part time Behavioural Interventionist with William Osler Health System. Provide critical support for individuals with responsive behaviours linked to Alzheimer's and other conditions.
As a key player on the Transitional Care Beds program, you will help unwinding complex behaviours and ensuring effective patient transition. You will identify triggers, implement tailored interventions, and educate caregivers and staff on best practices. The role is dedicated to fostering resilient and independent living for seniors, addressing their unique healthcare needs across various Osler locations.
Key Responsibilities:
• Conduct behavioural assessments following best practice guidelines
• Collaborate with caregivers to identify behaviour triggers
• Implement and document individualised behavioural interventions
• Educate hospital staff on managing responsive behaviours
• Support the transition of patients to their next care location
Requirements:
• Community college diploma in healthcare or related field
• One year of experience with seniors in acute care
• Certificate in gerontology preferred
• Excellent communication and interpersonal skills
• Ability to provide a gently persuasive approach
