Southbridge is seeking a passionate Registered Dietitian for a permanent, full time position. You'll be involved in nutritional assessments, personalized care planning, and ensuring the nutritional adequacy of seasonal menus. Engage with residents, families, and care teams to promote hydration and nutrition education.
Key Responsibilities:
• Consult on nutrition and hydration program needs
• Complete RAI MDS 2.0 assessments for residents
• Develop and evaluate individualized care plans
• Maintain communication with the Food Service Manager
• Review and approve seasonal menus for nutrition adequacy
Requirements:
• Registered Dietitian with College of Dietitians of Ontario
• 3-5 years’ experience in long-term care
• Proficient in Microsoft Office and RAI MDS 2.0
• Excellent organizational and communication skills
• Valid vulnerable sector check within the last 6 months
