This role reports directly to the Program Coordinator and is key in reducing emergency room visits while supporting individuals at risk of social isolation. You will play a central part in promoting client wellness through a holistic, multi-disciplinary approach, collaborating with healthcare providers and community organizations.
Key Responsibilities:
• Help clients engage with community resources
• Minimize hospital admissions through effective support
• Collaborate with primary care and hospital staff
• Support clients transitioning between care environments
• Address social isolation and improve quality of life
Requirements:
• Degree in Health or Social Sciences preferred
• 3 years' experience in health and social services
• Strong communication and interpersonal skills
• Ability to work independently and in teams
• Valid driver's license and vehicle access
