In this full time role, you will collaborate with both multidisciplinary teams and families in delivering essential care services. Your responsibilities include helping with hygiene, nutrition, and mobility while ensuring safety and monitoring wellbeing. Effective communication will play a crucial role in your interactions with patients and health professionals alike.
Key Responsibilities:
• Assist patients in their daily living activities across various settings
• Support nursing staff with routine care requirements




• Facilitate organized patient activities or programs
• Observe and report on patient mood and health status
• Maintain safety and communication within care teams
Requirements:
• Active registration or eligibility with the College of LPNs and HCAs
• Necessary certification issued within 18 months
• Satisfactory criminal record check mandatory
• Strong commitment to patient-centered care
• Excellent communication and teamwork abilities
