As a member of the multidisciplinary healthcare team, the Social Worker will apply their clinical knowledge and skills to provide comprehensive care, and follow-up to children ages 0-18.
Qualifications
- Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker.
- Master's Degree in Clinical Social Work from an accredited School of Social Work
- 3-5 years clinical experience (within the last 6 years) with children and families dealing with acute and chronic medical illness, death and dying, bereavement issues and chronic progressive illness
- Hospital experience is preferred
- Excellent knowledge of community support and placement services
- Knowledge of Healthcare Consent Act,
Substitute Decisions Act and Child and Family Services Act.
- Proven ability to work as a member of interdisciplinary healthcare teams.
- Demonstrated skills in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills
- Proven ability to work in a rapid-paced, complex environment and to organize, prioritize and problem solve effectively
- Demonstrated commitment to ongoing learning
- Strong interpersonal, communication, teamwork, consultation and teaching skills
