We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires a strong commitment to patient care, excellent communication skills, and the ability to work collaboratively within community setting. The Pharmacist will be responsible for providing pharmaceutical care, managing medication therapy, and educating patients and healthcare professionals on medication-related issues.
Duties
- Review and interpret medication orders to ensure accuracy and appropriateness.
- Dispense medications accurately while adhering to safety protocols.
- Provide patient counseling on medication usage, side effects, and interactions.
- Collaborate with healthcare providers to optimize medication therapy for patients.
- Monitor patient outcomes and adjust medication regimens as necessary.
- Maintain accurate records of medication administration and patient interactions.
- Stay current with medical terminology and advancements in pharmacotherapy.
- Participate in acute care settings, providing specialized services in pediatrics when needed.
- Educate staff and patients on best practices for medication management.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist license in the state of practice.
- Experience in a hospital setting is preferred, particularly in acute care or pediatrics.
- Strong knowledge of medical terminology and pharmacology.
- Excellent interpersonal skills with a focus on patient care and education.
- Ability to work effectively within a multidisciplinary team workplace.
- Commitment to continuous professional development and staying updated with industry standards.
