About the Role The Return to Work Case Manager will act as a central point of coordination between workers, employers, healthcare providers, insurers, and workers' compensation boards, when applicable.
Key Responsibilities Return to Work & Case Management
· Receive, review, and manage new return-to-work cases following injury, illness, medical leave, or workplace absence.
· Review medical notes, functional abilities information, job demands, and worksite realities to understand restrictions and capabilities.
· Develop secure and appropriate return-to-work plans, including modified duties, reduced or graduated hours, temporary restrictions, and accommodations.
· Ensure return-to-work plans align with documented restrictions, job demands, operational needs, and applicable duty-to-accommodate principles.
· Coordinate return-to-work timing, conditions, and follow-up communication with workers, employers, supervisors, human resources, healthcare providers, insurers, and WSIB/WCB representatives when applicable.
· Identify and help resolve medical, job-related, organizational, or personal barriers that may delay safe and sustainable return to work.
· Monitor return-to-work progress and adjust plans as required based on updated information, restrictions, or workplace needs.
· Maintain objective, accurate, and confidential case documentation in accordance with privacy and company recordkeeping expectations.

Consultation & Support
· Provide practical employer guidance regarding accommodation planning, restrictions, functional abilities, and return-to-work best practices.
· Support clients with WSIB/WCB-related return-to-work processes, documentation, case updates, and communication requirements.
· Prepare clear case summaries, status updates, and recommendations for clients, while avoiding diagnosis or treatment decisions outside scope.
· Help employers understand risk, compliance, and documentation considerations related to injury, illness, medical leave, and workplace reintegration.
· Provide support for substance use or substance abuse case management when required, including coordination of follow-up steps, referrals, and re-testing processes. Administrative & Team Responsibilities
· Maintain tracking systems, documentation control, and timely case notes.
· Participate in internal meetings, client discussions, and process improvement initiatives.
· Collaborate with NorthStream occupational health, testing, administrative, and management teams.
· Support policy, procedure, and workflow improvements related to return-to-work and disability management services.
· Communicate professionally through Microsoft Teams, email, phone, and assigned systems in accordance with the responsibilities of the role.
Qualifications & Requirements
· Post-secondary education in Disability Management, Occupational Health, Human Resources,
Nursing, Kinesiology, Rehabilitation, Social Work, Psychology, or a related field.
· Minimum 3 years of experience in return-to-work coordination, disability management, workers' compensation, occupational health, rehabilitation, claims management, or case management.
· Strong understanding of workplace accommodation principles and return-to-work best practices.
· Ability to interpret functional abilities information, workplace restrictions, and job demands in a practical and objective way.
· Excellent written and verbal communication skills, with the ability to build trust with workers, employers, healthcare providers, and client contacts.
· Strong organization, documentation, problem-solving, and time-management skills.
· Proficiency with Microsoft 365, including Word, Excel, Outlook, and Teams.
· Ability to maintain strict confidentiality, professionalism, and discretion at all times.
Considered
Assets
· Certified Disability Management Professional (CDMP), Certified Return to Work Coordinator (CRTWC), or related designation.
· Experience with WSIB, WCB, LTD, STD, or disability claims processes.
· Occupational health, rehabilitation, nursing, kinesiology, or workplace safety experience.
· Experience supporting safety-sensitive, industrial, unionized, or remote work environments.
· Knowledge of privacy, documentation, and confidentiality considerations in occupational health or disability management. Compensation
$62,000 - $90,000 annual salary, based on experience and qualifications.
