In this part time role, you'll manage patient care autonomously while collaborating with multi-disciplinary teams. This role focuses on delivering primary care and falls prevention services to the community.
Working 21 hours per week, you will address acute and chronic health conditions for diverse populations, including older adults. Your leadership in team-based care will support clients facing health inequities and ensure comprehensive health assessments align with community needs. Key Responsibilities:
- Serve as the Most Responsible Provider for a dedicated client panel
- Manage acute and chronic conditions across various age groups
- Collaborate with health professionals to deliver team-based care
- Conduct comprehensive health assessments and develop care plans
- Educate clients on disease prevention and promote health services Requirements:
- Master's degree in Nursing, CNO registration in Extended Class
- Minimum 2 years as a Nurse Practitioner, preferably in primary care
- Experience with priority populations, e.g., newcomers, older adults
- Technology proficiency, e.g., PS Suite EMR, Microsoft Office
- Excellent communication and interpersonal skills Drive change in community health as a dedicated Nurse Practitioner specializing in client-centered care at PQCHC.
