In this rewarding position, report to the Director of Care while providing personalized nursing support. You will assess residents, implement care plans, and coordinate with families and healthcare teams. Key Responsibilities:
- Implement and evaluate resident care/service plans
- Facilitate communication and coordination with healthcare providers
- Manage medication orders and document compliance
- Conduct follow-ups on resident/family interactions
- Collaborate on Infection Prevention and Control initiatives
Requirements:
- Active registration as a Registered Practical Nurse
- Experience with unregulated care providers preferred
- Strong oral and written communication skills
- Basic Cardiac Life Support certification necessary
- Recent Vulnerable Position/Sector Screening required
