In this rewarding position, report to the Director of Care while providing personalized nursing support. You will assess residents, implement care plans, and coordinate with families and healthcare teams. Your role enhances the quality of life through direct care, medication administration, and vital clinical assessments.
Key Responsibilities:
- Implement and evaluate resident care/service plans
- Facilitate communication and coordination with healthcare providers
- Manage medication orders and document compliance
- Conduct follow-ups on resident/family interactions
- Collaborate on Infection Prevention and Control initiatives
Requirements:
- Active registration as a Registered Practical Nurse
- Experience with unregulated care providers preferred
- Strong oral and written communication skills
- Basic Cardiac Life Support certification necessary
- Recent Vulnerable Position/Sector Screening required
