JOB SUMMARY:

Within the context of a agile patient/client/patient service culture, and in accordance with the established mission, vision, values and objectives of the organization, works under the general operational supervision of the Manager, with work direction from the Transition Services Coordinator in a team based environment and provides social support services to patients and related clerical support. Duties include interviewing /families/facilities for information, assisting to approach external resources, providing practical support to clients, arranges clients external appointments, and acquires and maintains a supply of up to date information on services/agencies/resources and community, government, health and social sector supports.

QUALIFICATIONS:

Education &

Experience

Graduation from a recognized certificate program such as Social Services Worker or Nursing Unit Assistant program or an equivalent combination of education, training.

Class 5 Drivers License and access to transportation.

Knowledge, Skills & Abilities

Ability to communicate effectively both verbally and in writing.

Develops and maintains positive and respectful relationships with co-workers and the multidisciplinary team.

Ability to deal with others effectively.

Physical ability to carry out the duties of the position.

Ability to organize work.

Ability to operate related equipment.





The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.
