In this advanced practitioner role, the SWOC Clinical Nurse Specialist leads wound and ostomy care programs by collaborating with interdisciplinary teams and community partners. You will provide specialized consultations to patients while demonstrating advanced nursing knowledge and clinical expertise.

Key Responsibilities:

- Act as an expert clinical resource for patients
- Develop and deliver care programs in collaboration with teams

Requirements:

- Bachelor's degree in a health-related field
- Registered Nurse with the College of Nurses of Ontario
- Minimum 3 years of experience as a Wound, Ostomy, and Continence nurse
- Completion of NSWOC or WOCN program
- Excellent communication and organizational skills
