Job Description: In this valuable role, the Clinical Supervisor is accountable for the supervision of community care assistants (CCA) and community care assistants in training (CCAIT) who are caring and making a difference for clients within the community.
The Clinical Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

Responsibilities
- Supervise and support CCA’s and CAIT’s to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
- Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
- Provide ongoing leadership through regular huddles, face‑to‑face meetings and one‑on‑ones.
- Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
- Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
- Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
- Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
- Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
- Time Type: Full time.
Qualifications
- Registration as a Licensed Practical Nurse in good standing.
- Minimum of 3‑5 years’ experience as a practicing nurse.
- Comprehensive patient health assessment skills are required.
- Possess an understanding of the principles of community nursing practice.
- Proven experience leading and managing a team.
- Knowledge of clinical documentation standards and practice guidelines is required.
- Excellent organizational, interpersonal, and teaching skills.
- Commitment to professional development in clinical practice that is relevant to home health care.
- Effective critical thinking and problem‑solving skills.
- Must possess a valid driver’s license and access to reliable transportation.
- Able to provide a current, clear Vulnerable Sector Screening (VSS).
