Deliver impactful clinical pharmacy services as a Clinical Pharmacy Services Provider. Focus on optimizing drug therapy and provide invaluable drug information to enhance patient care.
In this position, your role spans the provision of pharmaceutical care, addressing drug-related problems, and leading therapeutic plans in collaboration with healthcare professionals. You will actively participate in committees and quality initiatives while providing mentoring to pharmacy students and new staff members. Your ability to accurately validate and dispense prescriptions will enhance medication safety.
Key Responsibilities:
• Provide pharmaceutical care and assess drug-related issues
• Collaborate with healthcare professionals on therapy plans
• Offer drug information and patient education
• Participate in drug utilization and quality improvement projects
• Mentor and supervise students and new staff members
Requirements:
• Doctorate of Pharmacy preferred; certification required
• Licensed as a Pharmacist with Ontario College
• Relevant hospital pharmacy experience is beneficial
• Strong knowledge in pharmacodynamics and monitoring
• Excellent communication and leadership abilities
