Key Responsibilities:
• Provide outcome-based support as per individual plans
• Assist with personal care tasks and transportation needs
• Collaborate with FFS team members for consistent support




• Document daily activities accurately using case management software
• Stay informed of community resources for Service Users
Requirements:
• Two years post-secondary education in Human Services
• Current First Aid and CPI certification required
• Ability to lift 20 lbs and travel as needed
• Understanding of community programs and services
• Clear Criminal Records Check required
