CBI Home Health is seeking a committed Physiotherapist to join its Integrated Care Program. In this role, you will assist patients in rebuilding strength and functionality after hospitalization. Key Responsibilities:
• Deliver hands-on physiotherapy sessions in-home
• Evaluate and monitor patient progress and outcomes
• Collaborate with an interdisciplinary care team
• Educate patients and families on recovery strategies
• Maintain accurate documentation of client sessions
Requirements:
• Registered with the College of Physiotherapists of Ontario
• Valid driver’s license and insured vehicle for travel
• Demonstrated commitment to patient-centered care
