Job Summary

The Unit Clerk is required to perform their duties in a manner that isconsistent with the organization’s mission, vision and values. The Unit Clerk requires a high level ofefficiency, organization, confidentiality and time management skills whichare essential to carrying out the duties of this position. The Unit Clerk will provide clericalsupport to maintain a progressive, effective, organized healthcareenvironment that further enhances the patient and staff experience. The Unit Clerk operates, maintains andorders different types of office and clinical equipment and supplies forhospital units.

Key Accountabilities

- Performs allduties in accordance with the Mission Statement and policies and proceduresof St. Joseph’s Health Centre, and the Ministry of Health for Long Term Careguidelines.
- Demonstratescompetency skills for the following standards and indicators:

- Identifiesunsafe conditions or practices and takes appropriate action to address themincluding reporting or removing hazards

- Working withthe Interdisciplinary team to coordinate and organize day to day activitieson the unit
- Responsibleto answer telephone calls and fielding questions from patients, visitors,staff, volunteers.
- Ensuringthat information and messages are forwarded to the appropriate people.



Usesjudgment in interrupting for urgent inquiries
- Responsiblefor organizing patient admissions on the direction of the Interdisciplinaryteam. This includes communication withbusiness office; assigning an appropriate room based on insurance and patientneeds; communication and faxing of information required from referringpartners; photocopying of admission packages; chart assembly; completionpatient armbands and signs for room door and dining room tables
- Responsiblefor organizing patient discharges/deaths on the direction of theInterdisciplinary team. This includescommunication with Business Office; dismantling charts for Medical Records;faxing required information to Family Physicians and other community partners
- Monitoringof stationary supplies and re-ordering as necessary
- Filing ofpatient forms and reports in the correct health record after they have beenreviewed by the appropriate health professional
- Maintainingpatient filing system that facilitates accuracy and organization
- Photocopyingand faxing of forms and correspondence
- Distributingmail and correspondence
- Communicatevia telephone broadcast or email change in patient condition as required
- May assistin the completion of Physician/NP patient care orders in collaboration withteam leader or nurse processing order e.g. requisitions for scheduled labtests ordered, communication with interdisciplinary team as required; faxingappropriate information as required; informing interdisciplinary teammembers/physicians of consultations required etc.
- Workingtogether with the interdisciplinary team and families to identifytransportation needs and booking appropriate transportation as required e.g.family car, wheelchair taxi, stretcher, volunteer van etc.
- Maintaininglists required for communicating and collecting patient data – patientboards, etc.
- Coordinating/orderingspecialty mattress and other supplies. Collaborates with RN or NP
- Organizingstaff replacement as required when scheduler unavailable e.g. sick calls,transportation escorts, extra staff, education replacement etc.




- Workingtogether with the Interdisciplinary team to ensure the admission anddischarge MDS/NRS data is completed as expected including pulling charts frommedical records
- Other dutiesas assigned
- Covers otherunits as needed
- Contributesto a transparent culture of client and staff safety by adhering to andabiding by patient and staff safety policies and procedures set by SJHCG.
- Promotes anenvironment that encourages and supports change using change leadershiptheory.

Qualifications & Experience

- Post secondary diploma orcertificate from a recognized administrative program or equivalent required.
- Knowledge of medicalterminology required.
- Minimum one (1) yearsrecent clerical experience in a health care setting preferred
- Demonstrated computerskills with knowledge of Microsoft Office 365
- Demonstrated superiorwritten and verbal communication skills, and excellent interpersonal skills.
- Consistently demonstratesefficient problem solving
- Strong organizationalskills and the ability to prioritize required.
- Strong organizational,time-management, critical thinking and problem-solving skills
- Ability to function as ateam member in a collaborative environment required.
- Ability to acceptdirection and work independently required.
