Detailed Overview

As a member of the designated Community Mental Health & Substance Use Services or Integrated Response team, you will support engagement, care coordination and system navigation across substance use & mental health services, local service areas, and regional and provincial services, with an emphasis on reducing barriers to enter/re-enter the system of care and facilitating timely treatment and rapid re-engagement of clients where appropriate and feasible. You will provide client- and family-centered care through a trauma-informed approach and evidence-informed practice to individuals and families with and affected by serious and persistent mental disorders and alcohol and drug addictions. As a clinician in a shared-team model, you will provide initial screening, intake, client assessment, treatment recommendations and plans, determine referral needs, follow-up support, group education, liaison with community resources for specialized services, counsel clients, and outreach as required; where assigned to Aftercare services, you may be designated as the "most responsible clinician" for specific clients dependent on the client's primary needs.

You will maintain respect for client dignity and maximum client self-determination and independence in provision of services. You may participate in local committees and internal and external case conferences. You may develop and deliver in-services, presentations and training to clients, families and caregivers. You may participate in process improvement activities and follow clinical and customer best practices. You may be required to work at more than one location and/or on an outreach basis to engage with clients within their own environment and community.

Responsibilities





- Performs initial screening of referrals to determine appropriate treatment based on client needs, urgency of request and best practices.
- Supports the intake process and assesses client needs by gathering information via phone or other methods; conducts initial assessment of client mental health and substance use status and risk; collaborates with the interdisciplinary team to determine best approach to care and assess the need for emergent crisis intervention.
- Provides individual, group, and family support using a variety of interviewing, supportive counseling, and assessment methods; shares responsibility for workload to manage cases, formulate recovery plans and goals, provide crisis intervention, assist with home visits, run groups, and outreach as required.
- Provides first level intervention and support until formal treatment or further assessment can be conducted (e.g., short-term brief interventions, follow-up, monitoring, and evaluating treatment outcomes).
- Provides treatment/support and case management for complex situations including people in crisis; delivers individual and group therapy, develops recovery-oriented treatment options and plans, and collaborates with other team members on treatment decisions.
- Where longitudinal Aftercare services are required, designated clinicians provide treatment/support through case management and brokered connections to adjunct community services, supporting recovery and connection to the system of care pre- and post-treatment.
- Facilitates client utilization of appropriate community services and rehabilitative programs, liaises with service providers, and refers to other resources as required.
- Provides referrals to appropriate community treatment options and services, including residential programs, family therapy, counselling, addiction services, and other agencies; utilizes community resources to develop supportive and rehabilitative programs for clients and families.
- Advocates on behalf of clients by providing information about processes and available services; acts as a resource for clients and families.
- Provides short-term counseling to clients and families and offers information and guidance to other caregivers and health providers regarding program services, policies and procedures.
- Maintains client records in accordance with established standards and regulatory requirements,



ensuring information governance and confidentiality.
- Carries out responsibilities in health and safety compliance and reports unsafe situations as required.
- Develops linkages with other facilities and community resources to improve access and coordination of mental health services; maintains relationships with community physicians, psychiatrics, and other professionals.
- Maintains relevant data and statistics on intakes, referrals, and service requests; contributes to quality improvement and risk management activities.
- Participates in team planning, meetings, and organizational initiatives to support client, family, and caregiver goals.
- Engages in ongoing education to maintain current practice standards and knowledge of applicable legislation affecting care delivery.

Qualifications

Education, Training and Experience

Bachelor’s degree in Social Work, Clinical Counselling, Counselling Psychology or other relevant related discipline. Two (2) years’ recent, related experience working with clients and families with, or affected by, mental illness and substance use disorders, or an equivalent combination of education, training, and experience. Eligible for registration with applicable professional association or regulatory body. Valid B.C. Driver’s License and access to a personal vehicle for work-related purposes.

Skills And Abilities

- Demonstrated knowledge of harm reduction, trauma-informed, recovery-oriented approaches; concurrent disorders; addictions and addictions counselling.
- Demonstrated client assessment and case management skills.
- Effective communications, both verbal and written, including empathic and respectful communication.
- Knowledge of relevant Acts and regulations; ability to apply policies and procedures.
- Strong interpersonal skills and ability to collaborate with interdisciplinary teams and external partners.
- Ability to navigate community resources and coordinate referrals.
- Ability to communicate effectively in diverse and multicultural contexts.
- Ability to work with individuals, groups, and professionals to plan, implement, evaluate, and prioritize workloads.
- Ability to provide efficient service while meeting clinical and administrative requirements.
- Proficiency in computer use and data management tools.
- Physical ability to perform the duties of the position.
