As a full-time Dementia Care Clinician, you will leverage robust clinical skills in various settings, offering psychosocial assessments, supportive counselling, and collaborative care planning. This role not only focuses on direct client support but also involves working alongside interdisciplinary teams in Primary Care Memory Clinics, ensuring coordinated dementia care and advocacy for clients.
Key Responsibilities:
• Conduct psychosocial assessments for clients and care partners
• Develop tailored care plans to enhance quality of life




• Provide case management for complex care needs
• Collaborate with healthcare teams for integrated care
• Facilitate psychoeducational support groups for caregivers
Requirements:
• BSW or MSW from an accredited institution
• Registered Social Worker in positive standing
• Minimum three years of clinical experience
• Experience in dementia care and psychosocial assessments
• Valid Ontario driver’s licence and reliable vehicle
