In this role, you will coordinate care, perform assessments, and implement client-centered plans in a cooperative environment. Key Responsibilities:
• Provide psychiatric and general nursing care to clients
• Coordinate care as the primary worker for assigned clients
• Conduct assessments and develop inter-professional care plans




• Liaise with community agencies for effective care coordination
• Support clients’ successful integration into their communities
Requirements:
• Active member of the College of Nurses of Ontario
• Two years of nursing experience, preferably in mental health
• Strong skills in behavioral observations and assessments
• Ability to work collaboratively in a diverse team
• Bilingualism or proficiency in a second language is an asset
