Key Responsibilities:
• Assess individualized patient needs using appropriate tools

Requirements:
• CNO Registration as a Registered Nurse in good standing
• Valid driver’s license and vehicle access required
• Experience in community care is an asset
• Flexibility and reliability in scheduling
• Willingness to travel to patient residences
