As a First Link Coordinator, you will engage with clients, conduct cognitive assessments, and develop specialized educational programs. Your work will focus on advocating improved dementia services and organizing support groups for families affected by Alzheimer’s Disease and Related Dementias. This role is critical for fostering community support and requires a strong commitment to client well-being.
Key Responsibilities:
• Administer cognitive assessments using standardized tools
• Provide education and support through home visits or phone




• Maintain accurate and confidential client records
• Organize and facilitate support groups for caregivers
• Advocate for improved services as needed
Requirements:
• Post-secondary education in health/social services field
• 1 to 3 years experience with dementia support
• Knowledge of community health care agencies
• Current Standard First Aid/CPR certification
• Valid driver’s license and vehicle access
