Become a Social Prescribing Counsellor with Western Ottawa Community Resource Centre, focusing on health promotion and client support. This part-time, mobile role involves helping individuals access vital community resources.
In this position, you will work under the supervision of the Supervisor of Health Promotion and Outreach, providing crucial crisis intervention services and referrals. Your focus will be on connecting clients with additional health supports and promoting well-being initiatives in the community. With your expertise, you will empower clients and enhance their overall health through tailored services and resources.
Key Responsibilities:
• Provide initial contact and establish rapport with current clients
• Make referrals to health and social services
• Support clients in clinics and community settings
• Facilitate education and support groups
• Participate in advocacy regarding service gaps
Requirements:
• Bachelor of Social Work or equivalent education
• Minimum two years of experience in direct service
• Valid driver’s license and access to a vehicle
• Fluency in English required, French is an asset
• Membership in CRPO or the college of Social Workers
