As an essential member of the interprofessional healthcare team at Assisted Living Alberta, you will utilize your nursing and critical thinking skills to provide safe, patient-centred care. In this regular part time role, you'll assess, plan, and evaluate client needs, working in their residences or alternate care settings for optimal support.
Key Responsibilities:
• Assess and evaluate client needs for tailored care
• Coordinate quality care services in homes and facilities
• Utilize nursing processes and problem-solving skills
• Collaborate with the Registered Nurse Case Manager
• Document and monitor care plans effectively
Requirements:
• Accredited practical nurse education program completed
• Eligible for registration with CLHA
• Current Basic Cardiac Life Support certification
• Two years of recent Home Care experience
• Valid Driver's license and access to a vehicle
