As a Registered Dietitian (RD) for Extendicare, you'll provide essential nutrition and hydration care for residents, including assessments and personalized planning. Your expertise will guide interventions and monitoring for those at nutritional risk, while developing plans tailored to individual needs. You’ll also contribute to care conferences and committees, promoting high standards of practice.
Key Responsibilities:
• Assess nutrition status upon admission and annually
• Develop and modify nutrition care plans for residents
• Document resident nutrition care accurately




• Provide dietary education and counseling to families
• Monitor residents at high nutritional risk
Requirements:
• University degree in Food and Nutrition
• 1-2 years' experience in long-term care preferred
• Current registration with provincial Dietitian regulatory College
• Solid communication and critical thinking skills
• Proficiency in Microsoft Office and relevant software
