In this position, you'll deliver essential health services focused on mental health and substance use while working collaboratively within a multidisciplinary team. You will drive positive health outcomes by providing primary care and advocating for client needs, all within a harm reduction framework. Engage with individuals facing systemic barriers and contribute to health promotion strategies in the community.
Key Responsibilities:
• Assess and treat clients' health conditions
• Coordinate sexual health visits and screening clinics
• Advocate for clients in accessing health services
• Collaborate on program development and evaluation
• Educate clients about health and lifestyle issues
Requirements:
• Current registration as Nurse Practitioner with CNO
• Minimum of 2 years clinical experience
• Evidence of controlled substances education
• Experience in primary care/community settings is beneficial
• Strong interpersonal and advocacy skills
