Location: Southbridge Ottawa
Salary Range: $55,000 - $60,000
Status: Part‑time vacancy

Southbridge is thrilled to announce the opening of our newest long‑term care facility right in the heart of Ottawa. This 192‑bed long‑term care home, built with comfort and community at its core, will soon welcome its new residents this Fall 2025. Southbridge Ottawa is proud of our recent state‑of‑the‑art home and welcomes you to join our team!

Responsibilities

- Coordinating the completion of MDS assessment on all residents within established timelines
- Understanding how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes
- Understanding and providing critical analysis of data at different levels of aggregation (Resident, Home Health Zone, etc.)
- Articulating for staff the strengths of MDS as a clinical assessment tool
- Articulating the financial implications of coding inconsistencies
- Acting as key contact and liaison with: Health Zone, AH&W;, CIHI, PCC, etc.
- Developing facility‑specific material within the PCC program as required
- Responding to Health Zone on all submission errors
- Facilitating response and correction of submission errors with facility staff
- Coaching key facility personnel to build capacity for understanding RUG patient classification levels
- Developing and introducing processes, and systems to improve accuracy of RAI content




- Facilitating audits of RAI assessments for content accuracy
- Participating in strategic planning for program development/improvement
- Responsible for Clinical and Admin Setup role to support MDS within PCC software program
- Interpreting reports generated by Health Zone, AH&W; or CIHI and providing critical analysis for Home
- Understanding the development of RAP’s, quality indicators, and RUG case mix products

Qualifications

- 3–5 years experience in long‑term care, experience with MDS an asset
- Registered as a RPN under CNO
- Knowledge in nursing rehabilitation and working OT and PT
- Strong analytical and assessment skills
- Ability to lead training and education with adult learners
- Knowledge of InterRAI suite of tools, MDS/RUG III patient classification systems
- Effective communication skills
