Support mental health caregivers as a Crest Manager by coordinating care and managing outreach activities. Collaborate closely with the Head/Deputy Head to meet caregiver needs effectively.
In this pivotal role, you will lead engagement programs, manage clinic outposts, and ensure timely referrals for caregivers. Your responsibilities include developing individualized care plans, conducting assessments, and implementing psychosocial interventions. You will also work closely with Community Health & Engagement teams to monitor caregiver progress and address emerging risks.
Key Responsibilities:
• Coordinate internal and external referrals for caregiver support
• Conduct screenings and develop individualized care plans
• Facilitate engagement activities for caregivers in the community




• Monitor caregivers’ conditions and escalate concerns
• Contribute to data collection and service evaluation
Requirements:
• Degree in counselling, social work, or psychology preferred
• Minimum of 2 years experience in mental health
• Robust communication and organizational skills
• Passion for supporting families with mental health issues
• Dedication to continuous improvement in service quality
