WoodGreen seeks a passionate Crisis Specialist in Toronto to address the urgent needs of seniors aged 55-65 facing crises. You will engage in direct outreach, assessment, and service implementation while collaborating with multiple health and community agencies. This position demands a proactive approach in connecting clients with necessary resources and ensuring follow-up care.
Key Responsibilities:
• Participate in mobile crisis intervention and outreach
• Assess client needs and develop personalized service plans
• Facilitate access to emergency medical services




• Monitor client progress and re-engage necessary supports
• Refer clients to community resources and services
Requirements:
• MSW or postgraduate degree with 2+ years’ experience
• Registration with the Ontario College of Social Workers
• Proven crisis support experience with seniors
• Robust communication and assessment skills
• Ability to work independently and collaboratively
