About The Job

- Supports the client’s achievement of safe, realistic, and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client
- Develops, implements and adapts a care plan, delivers direct care,



assesses service eligibility and provides care management to help clients manage their own care and navigate through the various services available within the community setting
- Enhances quality of life for the client and family; consults, confers, and collaborates with other health care providers
- Demonstrates continuous improvement and evidence based practice in nursing practice
- Maintains and advances own clinical competence
- Participates on designated committees/teams and approved research projects as assigned

This is a Casual opportunity

Casual Employees are not guaranteed hours of work and can range from 0-37.5 hours per week on an as‑needed basis.

Shifts of work will be days rotating from 08:00 to 16:00 hours
