Enhance nutritional care as a Part Time Dietitian at St. Joseph’s Home Care within HNHB boundaries. Support diverse client needs while monitoring nutritional health and providing therapeutic education.
This part time role focuses on delivering quality nutritional care as part of the Integrated Comprehensive Care Program (ICCP). You will conduct nutritional assessments, educate clients and staff, and support interdisciplinary teams to optimize health outcomes. Responsibilities include developing individualized service plans and liaising with community resources to address food security and dietary needs.
Key Responsibilities:
• Perform nutritional assessments for program clients
• Educate clients on therapeutic and general nutrition
• Collaborate with the multidisciplinary healthcare team
• Document client issues and updates accurately
• Build relationships with community agencies for support
Requirements:
• University degree in related field required
• Completion of a dietitian internship required
• Updated registration with the College of Dietitians
• Minimum 3 years of similar experience preferred
• Previous work with vulnerable populations an asset
