Key Responsibilities:
• Assess patients' needs to create individualized treatment goals
• Recommend, assess, and adapt treatment activities and environments
• Facilitate discharge and transition planning for continued care




• Support nursing staff with supervision and guidance
• Promote ongoing learning and improvement among colleagues
Requirements:
• Master's Degree in Occupational Therapy required
• Registered with the College of Occupational Therapists of Ontario
• Experience with geriatric rehab and ADP certification preferred
• Valid driver’s license and reliable vehicle
• Successful completion of background checks and TB tests
