The Hamilton Family Health Team is looking for an Outreach Worker to enhance patient interactions and care support. You'll work in tandem with Social Workers to manage cases and execute personalized care plans. Applicants should exhibit an understanding of social determinants affecting health outcomes and possess experience in relevant health fields.
Key Responsibilities:
• Provide in-home and community support to patients
• Work closely with Social Workers on care plans
• Accompany patients to medical appointments
• Assess health equity challenges in patient care
• Apply de-escalation strategies when necessary
Requirements:
• Relevant Degree or Diploma in Social Work
• Significant experience in community health services
• Registered with a qualified college (preferred)
• Training in mental health or addictions is beneficial
• Familiarity with diverse community needs
