Reporting to the Community Outreach Team Leader, this position aids youth struggling to find necessary resources, especially those transitioning out of care. You'll develop tailored support plans, mentor youth through challenges, and work collaboratively within a team to foster positive community outcomes. Leadership and community development skills are essential in this impactful role.
Key Responsibilities:
• Implement assessments and develop support plans for youth
• Mentor youth facing housing, education, and health challenges




• Coordinate case management within the youth support framework
• Use motivational interviewing to guide youth effectively
• Assist with crisis intervention and outreach services
Requirements:
• Degree or diploma in human/social services required
• 3-5 years of youth work experience
• Proven ability in case management
• Excellent written and verbal communication skills
• French proficiency is an asset
