You will provide coordination and support to Indigenous patients, assisting them in accessing health services and addressing their concerns. Flexibility is required as you will work diverse hours across multiple sites and engage in an on-call rotation.
Key Responsibilities:
• Maintain confidentiality and promote inclusive care

Requirements:
• Four-year degree in Health or Social Sciences
• Three years in case management or navigation
• Lived experience as Indigenous
• Knowledge of Indigenous heritage and trauma
• Strong communication and problem-solving skills
